В исследование включены пациенты с нВМД и максимально корригированной остротой зрения (МКОЗ) менее 0,1, получавшие антиангиогенную терапию с нарушением режима лечения (13 глаз, группа I), не получавшие лечение (15 глаз, группа II) и пациенты, получавшие антиангиогенную терапию с нарушением режима лечения и последующим прекращением лечения (18 глаз, группа III). В начале и в конце периода наблюдения всем пациентам было выполнено стандартное офтальмологическое обследование и проведена оптическая когерентная томография (ОКТ).</p></sec><sec><title>Результаты</title><p>Результаты. В группе I за период наблюдения 38,9 ± 27,5 месяца было выполнено 7,6 ± 4,3 инъекции. Период наблюдения в группе II составил 31,7 ± 28,6 месяца. В группе III за период лечения 26,9 ± 26,4 месяца было выполнено 4,8 ± 2,2 инъекции, после остановки терапии пациенты наблюдались в среднем 24,6 ± 18,6 месяца. За период наблюдения ни одна из групп не показала статистически значимого изменения МКОЗ или повышения центральной толщины сетчатки (p &gt; 0,05). В начале периода наблюдения по данным ОКТ во всех трех группах с высокой частотой определялись тяжелые морфологические изменения в макуле в виде атрофии пигментного эпителия сетчатки (ПЭС), субретинального фиброза, разрыва ПЭС или гигантских интраретинальных кист.</p></sec><sec><title>Заключение</title><p>Заключение. У пациентов с нВМД, низкой остротой зрения и выраженными анатомическими изменениями в макуле проведение антиангиогенной терапии с нарушением режима не позволяет улучшить функциональный статус по сравнению с естественным течением заболевания. </p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Purpose</title><p>Purpose: To study long-term outcomes in neovascular age-related macular degeneration (nAMD) patients with low visual acuity and nonadherence to a treatment regimen of anti-vascular endothelial growth factor (VEGF) therapy.</p></sec><sec><title>Patients and methods</title><p>Patients and methods. In this study we included nAMD patients with best corrected visual acuity lower than 0.1 decimal equivalent, nonadherent to anti-VEGF therapy (13 eyes, group I), treatment-naïve (15 eyes, group II) or nonadherence to anti-VEGF therapy followed by termination of anti-VEGF treatment (18 eyes, group III). All participants received comprehensive ophthalmic examination and optical coherence tomography baseline and at the end of the follow-up period.</p></sec><sec><title>Results</title><p>Results: In group I, 7.6 ± 4.3 intravitreal anti-VEGF injections were performed over a mean period of 38.9 ± 27.5 months. In group II, a mean follow-up was 31.7 ± 28.6 months. In group III 4.8 ± 2.2 intravitreal anti-VEGF injections were performed over a mean period of 26.9 ± 26.4 months followed by treatment discontinuation for a mean period of 24.6 ± 18.6 months. None of study groups showed statistically significant changes in BCVA or an increase of central retinal thickness (p &gt; 0.05). At baseline all study eyes frequently demonstrated severe morphological changes in the macula including atrophy of retinal pigment epithelium (RPE), subretinal fibrosis, RPE tear, or giant intraretinal cysts.</p></sec><sec><title>Conclusion</title><p>Conclusion: In nAMD patients with low BCVA and severe anatomical changes in the macula nonadherence to a treatment regimen of anti-VEGF therapy results in non-superior long-term outcomes compared to natural course of the disease. </p></sec></trans-abstract><kwd-group xml:lang="ru"><kwd>антиангиогенная терапия</kwd><kwd>неоваскулярная возрастная макулярная дегенерация</kwd><kwd>оптическая когерентная томография</kwd></kwd-group><kwd-group xml:lang="en"><kwd>anti-VEGF therapy</kwd><kwd>age-related macular degeneration</kwd><kwd>optical coherence tomography</kwd></kwd-group></article-meta></front><back><ref-list><title>References</title><ref id="cit1"><label>1</label><citation-alternatives><mixed-citation xml:lang="ru">Нероев В.В.
