Оценить гипотензивный эффект модифицированной непроникающей глубокой склерэктомии. </p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Больные с первичной открытоугольной глаукомой, вошедшие в исследование, были разделены на 2 группы: основная (группа 1) — 29 человек (46,8%), у которых проведена модифицированная непроникающая глубокая склерэктомия, и группа сравнения (группа 2) — 33 пациента (53,2%), у которых была выполнена стандартная непроникающая глубокая склерэктомия. Тонометрическое давление у больных основной группы составило 31,9±3,2мм рт. ст., у больных контрольной группы — 31,5±3,3мм рт. ст. Модификация непроникающей глубокой склерэктомии заключается в отсепаровке П-образного склерального лоскута свободным краем к лимбу и основанием к экватору, вскрытии шлеммова канала, формировании склерального тоннеля, в который вставляют нить из полиамида 5/0, тем самым расширяя его. </p></sec><sec><title>Результаты</title><p>Результаты.Впервые 3 недели после операции у больных основной группы наблюдалась умеренная гипотония в пределах 15,7±1,56ммрт. ст., у пациентов группы сравнения — 16,9±1,9ммрт.ст. Через 3 месяца внутриглазное давление у больных основной группы составило 17,47±1,24мм рт. ст., а группы сравнения — 18,74±1,37мм рт. ст. Спустя 10 месяцев после операции тонометрическое давление у пациентов первой группы было достоверно ниже по сравнению со второй группой (р&lt;0,05), составив 19,14±1,27мм рт. ст. и 21,31±1,42мм рт. ст., соответственно. </p></sec><sec><title>Заключение</title><p>Заключение. Предложенная операция даёт стабильный и длительный гипотензивный эффект, позволяет уменьшить спаечный процесс в зоне фильтрации. Введённая в склеральный тоннель нить расширяет его и способствует постоянной фильтрации внутриглазной жидкости. </p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To analyze the efficacy of modified non-penetrating deep sclerectomy. </p></sec><sec><title>Materials and methods</title><p>Materials and methods. POAG patients enrolled in the study were divided into 2 groups: study group included 29 patients who were referred for modified non-penetrating deep sclerectomy, control group included 33 patients who were referred for standard non-penetrating deep sclerectomy. IOP level was 31.9±3.2 mm Hg in the study group and 31.5±3.3 mm Hg in the control group. Modification of non-penetrating deep sclerectomy consists in the separation of fornix-based Ushaped scleral flap, Schlemm’s canal opening, scleral tunnel creation and its dilation with polyamide 5/0 suture. </p></sec><sec><title>Results</title><p>Results. Three weeks after surgery, moderate hypotension was observed in both groups (15.7±1.56 mm Hg in the study group and 16.9±1.9 in the control group). Three months after surgery, IOP level was 17.47±1.24 mm Hg in the study group and 18.74±1.37 in the control group. Ten months after surgery, IOP level in the study group was significantly lower than in the control group (19.14±1.27 mm Hg and 21.31±1.42 mm Hg, respectively, р&lt;0.05). </p></sec><sec><title>Conclusions</title><p>Conclusions. The proposed surgical technique provides stable and long-term hypotensive effect and prevents excessive scarring in filtering area.
