Изучали системное артериальное давление у 84 пациентов (92 глаза) с возрастной макулярной дегенерацией, которых лечили интравитреальными инъекциями Эйлеа в фиксированном режиме.</p></sec><sec><title>Результаты</title><p>Результаты. Установлено, что достоверно чаще плохой ответ на лечение в виде частичной нереспонденции или прогрессирования заболевания, несмотря на лечение, ассоциируется с повышенными показателями диастолического артериального давления (ДАД, р = 0,01). Повышение систолического (САД) артериального давления у больных с артериальной гипертензией и нВМД сопровождается повышением глазного перфузионного давления (р &lt; 0,01), что, по-видимому, ухудшает всасывание ангиостатика и обусловливает плохой ответ на лечение. По результатам исследования наиболее благоприятный коридор значений САД, ассоциированный с идеальной респонденцией, находится в диапазоне значений 104–140 мм рт. ст., а ДАД — в диапазоне 68–80 мм рт. ст., что следует принять за рекомендуемые параметры АД у больных нВМД, получающих курс лечения Эйлеа. Внутриглазное давление (ВГД) продемонстрировало себя в качестве второго модифицируемого самостоятельного и независимого фактора риска плохого ответа на лечение нВМД препаратами 1-й линии анти-VEGF-терапии. Биомаркером, ассоциированным с идеальным ответом, был показатель ВГД 12,6 мм рт. ст., а коридор рекомендуемых значений — 11–21 мм рт. ст. Повышение офтальмотонуса с выходом персонифицированных значений за указанный коридор, по-видимому, ухудшит исход лечения.</p></sec><sec><title>Заключение</title><p>Заключение. Выявление модифицируемых факторов риска имеет чрезвычайно важное значение в практической офтальмологии, поскольку раскрывает возможность повышения шансов пациента на лучший исход лечения. Модифицируемые факторы риска — ценные действенные инструменты, пополняющие наш арсенал. Информация о них важна не только при лечении ВМД, но и может стать мотивацией пациента для перехода к здоровому образу жизни и снижению риска развития заболевания.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Purpose</title><p>Purpose: to assess hypertension as a risk factor for a poor response to antiangiogenic therapy.</p></sec><sec><title>Patients and methods</title><p>Patients and methods. Systemic blood pressure was studied in 84 patients (92 eyes) with age-related macular degeneration who were treated with intravitreal injections of Eilea in a fixed mode.</p></sec><sec><title>Results</title><p>Results. It was found that significantly more often a poor response to treatment in the form of partial non-resposing or progression of the disease, despite treatment, is associated with elevated diastolic blood pressure (DBP, p = 0.01). An increase in systolic (SBP) blood pressure in patients with arterial hypertension and AMD is accompanied by an increase in eye perfusion pressure (p &lt; 0.01), which apparently worsens the absorption of angiostatics and causes a poor response to treatment. According to the results of the study, the most favorable corridor of SBP values associated with ideal response is in the range of values of 104–140 mm Hg, and DBP is in the range of 68–80 mm Hg st., which should be taken as the recommended parameters of blood pressure in patients with nVMD receiving a course of treatment for Eylea. The identification of modifiable risk factors is extremely important in practical ophthalmology, as it opens up the possibility of increasing the patient’s chances of a better treatment outcome. Modifiable risk factors are valuable and powerful tools that replenish our arsenal.
