В исследовании принимали участие 2 операционные медсестры, каждая из которых подготовила по 20 образцов раствора цефуроксима для внутрикамерного введения по единому протоколу разведения. Концентрацию антибиотика в растворах измеряли фотоколориметрическим методом.</p></sec><sec><title>Результаты</title><p>Результаты. Эксперимент, моделирующий условия реальной клинической практики, продемонстрировал значительный размах концентраций цефуроксима в растворах, приготовленных ex tempore. Средняя концентрация антибиотиков в пробах, подготовленных 2 медсестрами, составляла 1,189 [1,075; 1,383] и 1,019 [0,984; 1,10] мг/мл соответственно, р = 0,0002. Размах колебаний составил 0,4 мг/мл, или 40,5 % от минимального зарегистрированного значения.</p></sec><sec><title>Заключение</title><p>Заключение. Высокая вариабельность концентрации цефуроксима при приготовлении ex tempore может приводить к ряду нежелательных явлений. Непреднамеренная передозировка обусловливает развитие серозной отслойки сетчатки и токсического синдрома передней камеры. Пониженное содержание антибиотика можно рассматривать как одну из причин развития инфекционных осложнений, вызванных чувствительными к цефуроксиму штаммами бактерий. Актуальной научной задачей представляется совершенствование подходов, субстанций и путей введения противомикробных средств, позволяющих повысить безопасность катарактальной хирургии в условиях меняющейся структуры и нарастающей резистентности глазной микробиоты.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Purpose</title><p>Purpose: to evaluate the variability of cefuroxime concentration for intracameral administration during preparation of ex tempore solutions under conditions of real clinical practice.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. Two operating room nurses participated in the study, each prepared 20 samples of cefuroxime solution for intracameral administration according to a uniform dilution protocol. The concentration of antibiotic in the solutions was measured by photocolorimetric method.</p></sec><sec><title>Results</title><p>Results. The experiment modelling the conditions of real clinical practice demonstrated a significant difference of cefuroxime concentrations in solutions prepared ex tempore. The mean antibiotic concentration in samples prepared by 2 nurses was 1.189 [1.075; 1.383] mg/ml and 1.019 [0.984; 1.10] mg/ml, respectively, p = 0.0002. The range of variation was 0.4 mg/ml, or 40.5 % of the minimum recorded value.</p></sec><sec><title>Conclusion</title><p>Conclusion. High variability of cefuroxime concentration during ex tempore preparation may lead to a number of undesirable events. Unintentional overdosing causes the development of serous retinal detachment and anterior chamber toxic syndrome. Low antibiotic content can be considered as one of the reasons for the development of infectious complications caused by cefuroxime-sensitive bacterial strains.
