Для «сухого глаза» адаптированы опросники, неспецифические в отношении конкретной нозологии. Кроме того, определена краткая форма изучения медицинских итоговых параметров (MOS-SF-36) и метод оценки полезности. Методы. При изучении экономических аспектов проблемы применены следующие методы: информационно-аналитический, математический, методы оценки технологий здравоохранения (моделирование, анализ бремени болезни, анализ влияния на бюджет). По результатам проведенного исследования построена модель «древо решений», использованная для расчета затрат на лечение пациентов с блефароконъюнктивальной и экзогенной формой синдрома «сухого глаза» при компьютерном зрительном синдроме. При построении модели рассмотрены 2 сценария ведения пациентов: типичная практика и лечение пациента с применением терапевтической гигиены век. Для пациентов в раннем послеоперационном периоде после проведения лазерного специализированного кератомилеза построена аналогичная модель. Анализ проводили на краткосрочный период (1 год). При проведении анализа учтены прямые медицинские затраты. При таком типе анализа подсчитывают полную стоимость («экономическое бремя») болезни. Для оценки распространенности учитывали данные о частоте встречаемости заболевания и официальные данные о численности взрослого населения в 2014 г. Анализ влияния на бюджет позволяет оценить в денежном выражении и отражает разность суммарных экономических эффектов от применения технологий сравнения. Анализ чувствительности позволяет оценить стабильность результатов, полученных при моделировании. В качестве наиболее значимого параметра для анализа была выбрана стоимость медикаментозного лечения. Результаты и выводы. Прямые медицинские затраты у пациентов при демодекозе в условиях типичной практики и применения терапевтической гигиены век составили 14623 руб. и 9200 руб., соответственно (экономия 37 %); при компьютерном зрительном синдроме — 17630 руб.и 9200 руб., соответственно (экономия 47 %); у пациентов после проведения лазерного специализированного кератомилеза — 4425,5 руб. и 3004 руб., соответственно (32 %). Анализ экономического бремени болезни с учетом распространенности заболевания и затрат на типичную практику ведения пациентов доказал высокий уровень изучаемого показателя для населения: в 2015 г. он составил 576 198 416 556 руб. При применении терапевтической гигиены век экономия прямых медицинских затрат составляет в среднем 44 %.</p></abstract><trans-abstract xml:lang="en"><p>The value of dry eye for general public health increases due to its high occurence in elderly persons and aging population. Non-specific questionnaires are adopted for dry eye. Additionally, 36‑item Medical Outcomes Study-Short Form (MOS-SF-36) and benefit evaluation method were developed. Methods. Information analysis and mathematical methods and public health technology evaluation methods (i.e., modelling, analysis of disease burden, and budget impact analysis) were applied to economic aspect study. Decision tree model based on these finding was developed and applied to cost estimating of blepharoconjunctival and exogenous dry eye treatment in computer vision syndrome. Two scenarios of patient management were considered, i.e., typical management and management using therapeutic eyelid hygiene. Similar model was developed for early postoperative period after LASIK. Short-run analysis (1 year) was performed, direct medical costs were considered. This analysis implies the calculation of overall cost (economic burden) of the disease. Official data on adult population size in 2014 and the occurrence of the disease were used to assess prevalence. Budget impact analysis evaluates the difference in total economic effects due to comparison technology use in money terms. Sensitivity analysis assesses the stability of simulation results. Medical treatment cost was considered the most valuable parameter. Results and conclusions. Direct medical costs of typical management and management using therapeutic eyelid hygiene in demodicosis, computer vision syndrome, and after LASIK were 14,623 RUB and 9,200 RUB (savings of 37 %), 17,630 RUB and 9,200 RUB (savings of 47 %), 4,425.5 RUB and 3,004 RUB (savings of 32 %), respectively. Analysis of economic burden with respect to disease occurrence and typical management costs demonstrated that in 2015 this parameter was estimated as 576,198,416,556 RUB.
