Лазеркоагуляция сетчатки (ЛКС) при пролиферативной диабетической ретинопатии (ПДР) является важной составляющей лечения на разных этапах, в том числе при проведении витреоретинальных хирургических операций. Вопрос оптимального объема интраоперационной ЛКС не имеет консенсуса, в связи с этим его уточнение является актуальной задачей.Цель — определить влияние объема интраоперационной ЛКС на частоту осложнений хирургического лечения ПДР.Пациенты и методы. Проведено проспективное исследование в трех сопоставимых по клинико-анамнестическим данным и объему лечения группах пациентов. Различие заключалось в объеме применения ЛКС: в группе 1 ЛКС не проводили, в группе 2 выполняли панретинальную ЛКС в четырех квадрантах на ближней и средней периферии сетчатки (1000–1500 коагулятов), в группе 3 — периферическую ЛКС от экватора сетчатки до ora serrata (600–800 коагулятов). Промежуточные результаты лечения оценивали на сроке 1 месяц от проведения операции, отдаленные — через 12 месяцев.Результаты. В раннем послеоперационном периоде статистически значимых межгрупповых различий не выявлено. В отдаленном периоде наблюдения в группе 1 определена более высокая частота развития гемофтальма и неоваскулярных осложнений в переднем сегменте глаза — рубеоза и неоваскулярной глаукомы. Неожиданным явилось увеличение частоты диабетического макулярного отека в отдаленном периоде после проведения панретинальной интраоперационной ЛКС. Наименьшая частота осложнений выявлена в группе 3 (после периферической ЛКС).Заключение. В рамках исследования получены и подтверждены ранее выявленные данные об увеличении риска развития послеоперационных осложнений — гемофтальма, рубеоза и неоваскулярной глаукомы — у пациентов, лечение которых не предусматривало проведение интраоперационной ЛКС. Наименьшая частота осложнений выявлена в группе после периферической ЛКС, что делает предложенный метод коагуляции перспективным в плане применения и дальнейшего изучения. Полученные данные являются перспективными в плане дальнейших патогенетических исследований, например с динамической оценкой цитокинового профиля витреальной полости при ПДР. Актуально и требует дальнейшего изучения также применение ингибиторов ангиогенеза и ЛКС интра- и периоперационно, в том числе в различных комбинациях.</p></abstract><trans-abstract xml:lang="en"><p>Relevance. Retinal laser photocoagulation (RLP) for proliferative diabetic retinopathy (PDR) is an important component of treatment at various stages including vitreoretinal surgery. The issue of the optimal intraoperative RLP area and localisation does not have a consensus; therefore, its clarification is an urgent task.Purpose: to evaluate the effect of intraoperative RLP area and localisation on the incidence of PDR surgical treatment complications.Patients and methods. The difference was in the use, area and localisation of RLP application: in group 1, RLP was not performed; in group 2, panretinal RLP was performed in four quadrants on the near and middle periphery (1000–1500 coagulates); in group 3, peripheral RLP was performed from the retinal equator to the ora serrata (600–800 coagulates). Intermediate treatment results were assessed within 1 month of surgery, and long-term results — after 12 months.Results. In the early postoperative period there were no statistically significant differences between groups. In the long-term follow-up period group 1 revealed a higher incidence of vitreous haemorrhage and anterior segment neovascular complications — rubeosis iridis and neovascular glaucoma. An unexpected increase in the incidence of diabetic macular edema in the long-term period after panretinal intraoperative RLP. The lowest frequency of complications was found in group 3 (after peripheral RLP).Conclusion. The study obtained and confirmed previously identified data on an increased risk of developing postoperative complications (vitreous haemorrhage, rubeosis iridis and neovascular glaucoma) in patients whose treatment did not include intraoperative RLP.
