Пациенты и методы. Проведен анализ хирургического лечения 33 пациентов (33 глаз) с сосудистым бельмом роговицы различной этиологии, которым в анамнезе была проведена кератопластика. Средний возраст больных составил 54,6 ± 15,5 года (от 26 до 79 лет). В первую группу вошли 15 пациентов (15 глаз), которым проводили первый этап кератопротезирования с формированием интрастромального кармана методом мануального расслаивания бельма под интраоперационным контролем ОКТ. Вторую группу составили 10 пациентов (10 глаз), которым выполняли первый этап кератопротезирования с формированием интрастромального кармана при помощи фемтосекундного лазера. Третью группу составили 8 пациентов (8 глаз), которым была выполнена сквозная ре-кератопластика. Результаты. Были получены сравнимые функциональные результаты в исследуемых группах, в том числе по количеству интра- и послеоперационных осложнений. Однако в первой и третьей группах было клинически значимо больше интраоперационных осложнений, чем во второй группе. Отмечено клинически значимое различие по количеству послеоперационных осложнений: во второй группе — 30,0 %, в первой — 46,7 % и в третьей — 62,5 %. У пациентов первой и второй групп имело место стабильное положение опорной пластины кератопротеза в различные сроки наблюдения, а после проведения второго этапа кератопротезирования — улучшение зрительных функций. Толщина сосудистого бельма оставалась стабильной в обеих группах на всех сроках наблюдения. Заключение. При проведении первого этапа кератопротезирования возможно формирование кармана как мануальным методом, так и с использованием фемтосекундного лазера. Вариантом выбора у пациентов с сосудистым бельмом является сквозная кератопластика, однако следует учитывать высокий риск декомпенсации трансплантата после каждой последующей пересадки, в особенности у пациентов с ожоговой этиологией сосудистого бельма.</p></abstract><trans-abstract xml:lang="en"><p>Objective: To evaluate clinical and functional results of the surgical treatment in patients after ineffective penetrating keratoplasty. Patients and methods. The study enrolled 33 patients (33 eyes) aged from 26 to 79 years after ineffective penetrating keratoplasty. The first group included 15 patients (15 eyes), who underwent the first stage of keratoprosthesis implantation using manual technique of creating the intrastromal pocket under intraoperative AS-OCT control. The second group included 10 patients (10 eyes), who underwent the first stage of keratoprosthesis implantation with creating the intrastromal pocket using a femtosecond laser. The third group included 8 patients (8 eyes), who underwent penetrating rekeratoplasty. Results. Functional results in the study groups were comparable. There were no significant differences in the number of intra- and postoperative complications between three study groups (p &lt; 0.05). However, in the first and third groups, there were more intraoperative complications than in the second group. The significant difference in the number of postoperative complications was also noted: in the first group — 46.7 %, in the second group — 30.0 % and in the third group — 62.5 %. In the first and second groups, the position of the keratoprosthesis intralamellar plate was stable at different follow-up periods. Also, in these groups, after the second stage of keratoprosthesis implantation, vision improved. Vascular leucoma thickness was stable in both groups at all follow-up periods. Conclusion. The first stage of keratoprosthesis implantation can be performed using manual technique of creating the intrastromal pocket or using a femtosecond laser.
