Хирургические методы зависят от стадии КК и скорости его прогрессирования. На начальных стадиях наиболее часто применяются кросслинкинг роговичного коллагена (КРК) с различными модификациями протоколов облучения; интраламеллярная кератопластика (ИЛКП) с имплантацией роговичных сегментов (ИРС) или колец; на более поздних стадиях возможно проведение сквозной (СКП) или передней послойной кератопластики (ППК). Однако, несмотря на большой арсенал методов лечения КК, далеко не многие являются персонализированными и помогают достичь высоких функциональных результатов. Нерешенной является проблема остаточных аметропий и неправильного астигматизма, в связи с чем разработка новых подходов к хирургическим методам лечения КК остается актуальной по сегодняшний день.</p></abstract><trans-abstract xml:lang="en"><p>Keratoconus (KC) is a chronic progressive disease of the cornea characterized by thinning and cone-shaped protrusion, leading to a significant reduction in visual acuity. It is often difficult to correct optically using glasses or soft contact lenses. Surgical methods depend on the stage of KC and the rate of its progression. In the early stages, corneal collagen crosslinking (CXL) with various protocol modifications is most used, along with intralamellar keratoplasty (ILKP) with the implantation of corneal ring segments (ICRS) or rings. At later stages, penetrating (PKP) or deep anterior lamellar keratoplasty (DALK) may be required. However, despite the wide range of treatment options for KC, few methods are personalized and achieve high visual acuity outcomes.
