Представленные данные демонстрируют значительные диагностические трудности, связанные с отсутствием патогномоничных лабораторных маркеров ББ и необходимостью комплексной оценки клинико-анамнестических данных. Ключевую роль сыграло выявление у пациентки из эндемичного для ББ региона (Астраханская область) аллеля человеческого лейкоцитарного антигена (Human Leukocyte Antigen, HLA) B51 — генетического маркера предрасположенности к ББ. Учитывая резистентность к глюкокортикоидам и высокий риск необратимой потери зрения, была инициирована генно-инженерная биологическая терапия ингибитором фактора некроза опухоли (ФНО)-α адалимумабом. В результате лечения достигнута положительная динамика с полным восстановлением остроты зрения, регрессом макулярного отека и признаков нейроретинита по данным оптической когерентной томографии. Наблюдение подчеркивает важность включения ББ в круг дифференциальной диагностики рецидивирующих увеитов у молодых пациентов, значение генетического тестирования с выявлением HLA-B51 и целесообразность раннего назначения современных таргетных препаратов, прежде всего ингибиторов ФНО-α, для сохранения зрительных функций. Ключевым фактором успешного лечения в данном наблюдении явилось скоординированное взаимодействие специалистов офтальмологического и ревматологического профилей.</p></abstract><trans-abstract xml:lang="en"><p>The article analyzes the clinical observation of a 22-year-old patient with severe recurrent bilateral uveitis complicated by neuroretinitis, probably caused by Behcet’s disease. The presented data demonstrate significant diagnostic difficulties associated with the lack of pathognomonic laboratory markers of Behcet’s disease and the need for a comprehensive assessment of clinical and anamnestic data. The key role was played by the identification of the Human Leukocyte antigen (HLA) B51 allele, a genetic marker of predisposition to Behcet’s disease, in a patient from the Astrakhan region. Taking into account the resistance to glucocorticoids and the high risk of irreversible vision loss, a genetically engineered biological therapy with tumor necrosis factor (TNF)-alpha inhibitor adalimumab was initiated. As a result of the treatment, positive dynamics was achieved with complete restoration of visual acuity, regression of macular edema and signs of neuroretinitis according to optical coherence tomography. The observation highlights the importance of including Behcet’s disease in the differential diagnosis of recurrent uveitis in young patients, the importance of genetic testing for HLA-B51, and the expediency of early prescribing modern targeted drugs, primarily TNF-α inhibitors, to preserve visual functions.
