Изучить непосредственные и отдаленные результаты модифицированной синусотрабекулотомии (СТТ) при первичной открытоугольной глаукоме (ПОУГ).</p></sec><sec><title>Пациенты и методы</title><p>Пациенты и методы. В исследование вошли 42 больных (47 глаз) с ПОУГ I‑III стадии в возрасте 47‑85 лет (средний возраст — 67,5 лет). Женщин было 27 (64,3 %), мужчин — 15 (35,7 %). I стадия глаукомы диагностирована в 17,0 % (8 глаз), II стадия — в 44,7 % (21) и III — в 38,3 % (18) случаев. В среднем уровень офтальмотонуса на медикаментозном режиме составил 29,2±1,5 мм рт. ст. (при исследовании с помощью пневмотонометра). Всем пациентам до и после операции было проведено стандартное офтальмологическое обследование. Отдаленные результаты в сроки от 6 до 24 месяцев (в среднем — через 13,1 месяцев) были прослежены у 39 больных (43 глаза).</p></sec><sec><title>Результаты</title><p>Результаты. Из ранних осложнений гифема имела место в 10,6 % случаев, цилиохориоидальная отслойка — в 6,4 %. В отдаленные сроки нормализация ВГД в целом была достигнута в 93,0 % случаев, из них в 7,0 % случаев — на фоне дополнительной терапии. Острота зрения в отдаленные сроки сохранилась в 88,4 % случаев, ее ухудшение отмечено в 11,6 % и было связано с прогрессированием глаукомы в 7,0 % глаз и в 4,6 % — с помутнением хрусталика. Поле зрения осталось на дооперационном уровне в 90,7 % случаев и сузилось — в 9,3 %, главным образом, вследствие прогрессирования глаукомного процесса.</p></sec><sec><title>Заключение</title><p>Заключение. Предложенная крестообразная СТТ с фиксацией верхушки склерального лоскута в супраувеальном пространстве в изученные отдаленные сроки (до 2 лет) при ПОУГ обладает достаточно высоким и устойчивым гипотензивным эффектом (93,0 %) и относительно небольшим числом осложнений (особенно ЦХО — всего в 6,4 %) и катарактогенным эффектом (в 4,6 %). На наш взгляд, данная операция показана к использованию при I‑III стадиях ПОУГ, когда степень ожидаемого послеоперационного рубцевания невелика.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Purpose</title><p>Purpose: To study the short- and long-term results of modified sinus trabeculotomy (STT) in primary open-angle glaucoma (POAG).</p></sec><sec><title>Patients and methods</title><p>Patients and methods: We observed 42 patients (47 eyes) with POAG at 1‑3 stages aged 47‑85 (average age — 67.5 years). There were 27 women (64,3 %) and 15 men (35,7 %). The I stage of glaucoma was diagnosed in 17.0 % (8 eyes), II stage — in 44.7 % (21 eyes) and III stage in 38.3 % (18 eyes) patients. The average level of intraocular pressure (IOP) at the drug therapy was 29.2±1.5 mmHg (results obtained by pneumotonometer). All patients underwent a routine ophthalmic examination before and after surgery. Long-term results for period of 6‑24 months (13.1 months average) were assessed in 39 patients (43 eyes).</p></sec><sec><title>Results</title><p>Results. The most common early complications were hyphema (10.6 % patients) and ciliochoroidal detachment (6.4 % patents). Long-term results showed normalization of IOP in 93.0 % cases including 7.0 % against the background of additional therapy. Visual acuity preserved in 88.4 % cases, its reduction was observed in 11.6 % and was associated with the progression of glaucoma (7.0 %) and lens opacity (4.6 %). The field of view remained at the preoperative level in 90.7 % patients and narrowed in 9.3 %, mainly due to the progression of glaucoma process.</p></sec><sec><title>Conclusion</title><p>Conclusion. Cruciate STT with anchoring of the scleral flap apex in suprauveal space in POAG in long-term results (up to 2 years) has a sufficiently high and sustained hypotensive effect (93.0 %), cataractogenic effect (4.6 %) and a relatively small number of complications (especially in case of ciliochoroidal detachment — just 6.4 %).
