Усовершенствовать метод лечения начальных проявлений инфекционных язв роговицы на основе микродиатермо-коагуляции (МДК) в сочетании с наружной аутоцитокинотерапией (ЛЭАЦКТ), а также противовирусной и/или антибактериальной терапией. Пациенты и методы. В исследовании принимали участие 2 группы пациентов (всего 112 пациентов (112 глаз)), которые амбулаторно получали консервативное лечение от 7 дней до 1.5 месяцев при отсутствии положительной или с отрицательной динамикой. В группу I вошли 70 пациентов (70 глаз) с поверхностными герпетическими язвами роговицы, получавших противовирусную терапию (Полудан в инстилляциях и периокулярных инъекциях, мазь Зовиракс 3%); в группу II вошли 42 пациента (42 глаза) с поверхностными гнойными язвами роговицы (ГЯР), получавших инстилляции современных антибиотиков, антисептиков, противогрибковых препаратов. результаты и заключение. Использование комбинации МДК и наружной ЛЭАЦКТ является наиболее эффективным методом лечения пациентов с длительно незаживающими, торпидными герпетическими язвенными кератитами и позволяет достоверно сократить сроки излечения с 24,1±2,2 дней (при проведении одного лишь консервативного лечения) до 9,2±1,3 (при использовании обоих методов на фоне лекарственной противовирусной терапии). Применение МДК является методом выбора в лечении начальных стадий гнойной язвы роговицы в условиях амбулатории. Срок лечения достоверно (р&lt;0.05) сокращается с 25,6±3,2 дней (при использовании медикаментозной терапии) до 18,6±1,9 (сочетанное применение антибактериальной терапии и МДК). МДК — высокоэффективный, ургентный, доступный в амбулаторных условиях метод лечения герпетического кератита и начальных стадий гнойных язв роговицы. Метод наружной ЛЭАЦКТ оказывает выраженное противовоспалительное и регенераторное действие. Комбинация наружной ЛЭАЦКТ и МДК оказывает мощное синергетическое воздействие и позволяет сократить сроки лечения в 2 раза. Метод МДК, комбинация МДК+наружная ЛЭАЦКТ при раннем применении позволяют избежать проведения кератопластики у большинства больных с герпетическим кератитом и начальными стадиями гнойных язв роговицы.</p></abstract><trans-abstract xml:lang="en"><p>Aim. To improve the treatment of early infectious corneal ulcers by combining microdiathermocoagulation (MDC), external autocytokinotherapy, and antiviral and/or antibacterial therapy. patients and methods. The study enrolled 2 groups of outpatients (a total of 112 patients, 112 eyes) who either showed no improvement or deteriorated under 7-day to 1.5-month therapeutic treatment. Group I included 70 patients (70 eyes) with superficial corneal ulcers due to herpes virus infection under antiviral therapy (instillations and periocular injections of Poludan (PolyA:PoliU) as well as Zovirax 3% ophthalmic ointment), group II — 42 patients (42 eyes) with early-stage purulent corneal ulcers under instillations of modern antibiotics (current-generation fluoroquinolones). results and discussion. The combination of MDC and external autocytokinotherapy is the most effective treatment for torpid herpetic ulcerative keratitis that allows a reliable reduction in the recovery period: from 24.1±2.2 days (therapeutic treatment only) down to 9.2±1.3 days (both methods plus antiviral therapy). Moreover, MDC is the treatment of choice in outpatients with purulent corneal ulcer in its early stage. The healing period in this case can be also reliably reduced (р&lt;0.05) from 18.6±1.9 days (MDC plus antibacterial therapy). Hence, MDC is a highly effective urgent method of treatment available to ambulatory care patients with herpetic keratitis and early-stage purulent corneal ulcers. External autocytokinotherapy shows a pronounced anti-inflammatory and regenerative effect. When applied together, MDC and external autocytokinotherapy act synergistically and provide twice as short treatment periods.
