Выделены одна основная (58 пациентов) и две контрольные группы (78 и 83 пациента). Операция по факоэмульсификации катаракты в сочетании с вакуумной трабекулопластикой ab interno выполнена у 58 пациентов основной группы. Всем пациентам в до- и послеоперационном периоде проводили контроль динамики остроты зрения, показателей гидродинамики, а также оценивали глубину и величину угла передней камеры с помощью данных оптической когерентной томографии.Результаты: В позднем послеоперационном периоде в основной группе отмечено стойкое снижение офтальмотонуса с сохранением эффекта при наблюдении в течение 2‑х лет. Факоэмульсификация с имплантацией ИОЛ у больных с катарактой и ПОУГ способствует изменению анатомо-топографических показателей структур переднего сегмента глаза за счет достоверного увеличения глубины передней камеры на 1,2 мм и расширения угла на 3,3°. Дополнительное проведение вакуумной трабекулопластики ab interno в большей степени изменяет глубину передней камеры (на 1,8 мм) и расширяет ее угол (на 15,9°). Частота операционных осложнений комбинированного способа (вакуумной трабекулопластики ab interno и факоэмульсификации катаракты) составила 8,5 %, при этом в контрольных группах этот показатель равен, соответственно, 5,1 % и 8,4 %. Сроки госпитализации больных в стационаре во всех группах наблюдения между собой существенно не отличались.Заключение: Разработанная новая методика одномоментной вакуумной трабекулопластики ab interno и факоэмульсификации катаракты с имплантацией ИОЛ обладает гипотензивной эффективностью (74 %) в течение двух лет наблюдения. В послеоперационном периоде после данной операции в большинстве случаев возможна полная отмена гипотензивного медикаментозного режима или значительное уменьшение его интенсивности. Факоэмульсификация с имплантацией ИОЛ в сочетании с вакуумной трабекулопластикой ab interno менее травматична, чем используемые методы хирургического лечения катаракты и глаукомы, и показана при сочетании катаракты различной степени зрелости с первичной открытоугольной медикаментозно компенсированной и субкомпенсированной глаукомой.</p></abstract><trans-abstract xml:lang="en"><p>Aim. To improve the efficacy of cataract and primary open-angle glaucoma (POAG) surgery using a novel combined surgical technique.Materials and methods. 219 patients were included in the study. According to the design, all subjects weredivided into study group (n = 58) and two control groups (n = 78 and n = 83). Combined phaco and vacuum trabeculoplasty (VTP) ab interno was performed in 58 study group patients. Pre-op and post-op, all patents had a complete eye examination including vision and hydrodynamics testing as well as anterior chamber depth (ACD) and angle assessmentusing optical coherent tomography.Results. In study group, stable IOP decrease was observed for 2‑yrs follow-up. Phacoemulsification in cataract and POAG patients has an impact on anterior segment anatomy and topography owing to significant increase of ACD by 1.2 mm and anterior chamber angle by 3.3°. VTP ab interno provides additional ACD increase by 1.8 mm and anterior chamber angle by 15.9°. Complication rate was 8.5 % in study group (i.e., following combined phaco and VTP ab interno) and 5.1 % and 8.4 % in control groups. Hospitalization days in all groups had no significant differences.Conclusion. A novel method of combined VTP ab interno and cataract phacoemulsification provides significant IOP decrease (by 74 %) at 2‑yrs follow-up. Following VTP and phaco, fewer or even no hypotensive eye drops were required. This technique is less traumatic than traditional cataract and glaucoma surgery.
