Оценка информативности разработанного авторами метода калиброметрической оценки сосудов сетчатки (СС) для выявления гипертонической ангиопатии.Методы. В исследование вошло 146 человек (292 глаза) с артериальной гипертензией (АГ). Всем проводилось фотогра- фирование глазного дна с последующим калиброметрическим анализом. Оригинальный метод калиброметрии, предложен- ный авторами, основывается на полностью автоматизированном распознавании СС с расчетом центральных артериолярного и венулярного эквивалентов сетчатки (central retinal arteriolar equivalent — CRAE, central retinal venular equivalent — CRVE) и артериовенозного соотношения (AVR — arterio-venous ratio). Предварительно производилась оценка воспроизводимости, чувствительности и специфичности метода.Результаты. Метод калиброметрии СС показал себя как хорошо воспроизводимый. Чувствительность и специфичность метода составили 74% и 80,77%, соответственно. Сравнение с офтальмоскопической оценкой калибра СС позволяет делать вывод о лучшей прогностической силе метода калиброметрии (AUC 0,85 и 0,903, соответственно). Выявлено сужение арте- риол и уменьшение AVR с возрастом и ростом среднего артериального давления (АД). Венулы имеют тенденцию к сужению с возрастом, связанного с АД увеличения калибра венул выявлено не было.Заключение. При анализе группы больных АГ метод калиброметрии СС показал себя как информативный. Полученные результаты согласуются с данными ряда крупных популяционных исследований.</p></abstract><trans-abstract xml:lang="en"><p>Purpose: to evaluate the diagnostic capability of automated retinal vessels (RV) caliber estimation for hypertensive angiopathy.Methods: this study included 146 patients (292 eyes) with arterial hypertension. All the subjects underwent fundus photography and RV caliber estimation. the latter was performed using newly developed computer-based method for automated vessel detection and central retinal arteriolar and venular equivalents determination (CRAE &amp; CRVE). Sensitivity, specificity, and reproducibility of the method were estimated.Results: the method of RV caliber assessment showed good reproducibility. the overall specificity and sensitivity were 74% and 80.77%, respectively. Computer-based method of retinal vascular caliber assessment revealed higher predictive value comparing with ophthalmoscopic assessment (AUC = 0.903 and 0.85, respectively). Retinal arteriolar and venular caliber and AVR tend to decrease with age.
