Гиперрефлективные фокусы как биомаркер оптической когерентной томографии при заболеваниях сетчатки: патогенез, визуализация и клиническое значение

Аннотация

Представленный литературный обзор посвящен важному ОКТ-биомаркеру заболеваний сетчатки — гиперрефлективным фо- кусам (ГРФ); которые рассматриваются как перспективный индикатор воспалительных процессов и сосудистых нарушений сетчатки. Целью является уточнение современных представлений о природе и классификации гиперрефлективных фокусов как важного диагностического маркера; что необходимо для повышения точности диагностики и мониторинга наиболее распространенных заболеваний сетчатки; а также для разработки персонализированных подходов к лечению; в том числе с использованием технологий искусственного интеллекта в офтальмологии. ГРФ определяются как дискретные точечные или округлые структуры с повышенной оптической плотностью размером от 20 до 100 мкм; локализованные в пределах слоев сетчатки или в субретинальном пространстве. ГРФ являются не специфическим морфологическим признаком; а универсальным фенотипом; отражающим широкий спектр патофизиологических процессов. В статье подробно описана морфология фокусов; патогенетические механизмы их образования; прогностическая роль и отличия при возрастной макулярной дегенерации; диабетической ретинопатии; диабетическом макулярном отеке. Правильная дифференциальная интерпретация ГРФ требует комплексного подхода; включающего анализ анамнеза и факторов риска; детальную оценку их локализации и морфологических характеристик; а также учет сопутствующих ОКТ-признаков и данных мультимодальной визуализации. Интеграция этих параметров позволяет точно определить природу гиперрефлективных фокусов; выбрать оптимальную тактику ведения пациента и обоснованно прогнозировать течение заболевания.
