Вопросы трофической поддержки в детской офтальмологии

Аннотация

В практике врача-офтальмолога периодически возникают вопросы о целесообразности применения витаминов, микроэлементов и других нутриентов для профилактики и лечения различных глазных заболеваний. Это обусловлено значимостью органа зрения для социализации современного человека, а также напряженностью использования зрительного анализатора как в процессе выполнения профессиональных обязанностей, так и в бытовой деятельности. Особенно важны вопросы такой трофической поддержки в педиатрической офтальмологии, поскольку в настоящее время формирование органа зрения у детей идет в условиях повышенной зрительной нагрузки. Для населения, в том числе детского, индустриально развитых стран наиболее типичной формой дефицита витаминов является субнормальная обеспеченность ими, которая встречается среди практически здоровых детей различного возраста. Вопросы трофической поддержки у детей должны рассматриваться в связи с определенными видами офтальмопатологии. Самой распространенной проблемой среди пациентов детского возраста является миопия с прогнозируемым высоким ростом масштабов этой аномалии рефракции в будущем. В патогенезе многих дистрофических заболеваний, в том числе миопии, большое значение имеет оксидативный стресс, запускаемый как эндогенными процессами, так и различными внешними факторами, в том числе ультрафиолетовым излучением. Кроме витаминов с антиоксидантными свойствами (А, С, Е), для стабилизации миопии и профилактики развития дегенеративных заболеваний во взрослом возрасте имеют значение микроэлементы (цинк и медь), биофлавоноиды (антоцианозиды) и ксантофильные пигменты (лютеин, зеаксантин). Эти вещества участвуют в важнейших физиологических и биосинтетических процессах склеры, сетчатки и других структур глаза, оказывают влияние на его микрососуды и на окулярную гемодинамику в целом. Все эти вещества не синтезируются в организме человека, и для поддержания метаболизма и гомеостаза должно быть обеспечено достаточное поступление их в организм алиментарным путем с раннего возраста. Особый интерес представляют комплексные препараты, в которые включены полипотентные и синергичные действующие вещества.
